To perform a perfect counter attack in Middle Earth: Shadow of War press the counter button as the counter symbol is highlighted just before an enemy is about to attack you:

Press the counter button just before an enemy is about to hit you to get a perfect counter.

Keep an eye out for enemies that are about to attack you and prepare to wait for a short period before pressing the counter button.
